We are seeking a highly motivated and empathetic Health Coach to join our remote team. In this role, you will provide personalized, evidence-based coaching to members primarily through English consultations, helping them manage chronic conditions, achieve wellness goals, and adopt sustainable healthy habits. This is a 100% remote position, allowing you to work from the comfort of your home and make a significant impact on individuals’ lives across various time zones. You will leverage Livongo’s integrated platform to deliver high-quality, engaging, and effective coaching sessions.

Responsabilidades Clave

• Conduct one-on-one virtual coaching sessions in English with members to develop personalized health plans.
• Utilize motivational interviewing techniques to empower members to set and achieve health goals related to nutrition, physical activity, stress management, and chronic condition management.
• Monitor member progress, provide ongoing support, and adjust coaching strategies as needed.
• Educate members on evidence-based health practices and the effective use of Livongo's digital tools and resources.
• Maintain accurate and confidential records of member interactions and progress within our secure platform.
• Collaborate with a multidisciplinary team to ensure a holistic approach to member care.
• Participate in ongoing training and professional development to stay current with health coaching best practices.
• Adhere to professional ethical guidelines and maintain strict confidentiality of all member information.

Habilidades Requeridas

• Certification from an accredited Health Coach program (e.g., NBHWC, ACE, AFAA, WELCOA).
• Excellent verbal and written communication skills in English, suitable for professional consultations.
• Strong understanding of behavior change theories and motivational interviewing techniques.
• Proficiency in using digital health platforms and virtual communication tools.
• Empathy, active listening, and the ability to build rapport with diverse individuals.
• Ability to work independently, manage a caseload, and prioritize tasks effectively in a remote environment.
• Strong organizational skills and attention to detail.

Cualificaciones Preferidas

• Bachelor's or Master's degree in a health-related field (e.g., Public Health, Nutrition, Exercise Science, Psychology).
• Experience working with individuals managing chronic conditions such as diabetes, hypertension, or weight management.
• Familiarity with HIPAA or GDPR compliance in a healthcare context.
• Experience in a virtual or tele-health coaching role.
• Additional language proficiency is a plus, but English fluency is essential for this specific role.
